Please note that the required documents may vary depending on whether you are a new client or a returning client of our firm.

Document Requirements

New Clients:

This checklist applies to new clients preparing their income tax return with our firm for the first time. Required documents may vary for returning clients.

Personal Information

  • Government-issued photo ID (taxpayer and spouse, if filing jointly)
  • Social Security cards or ITIN letters for:
    • Taxpayer
    • Spouse
    • Dependents
  • Birth Certificate for all dependents
  • Bank account and routing number (for direct deposit or payment)

Income Documents

Provide all forms received, including but not limited to:

  • W-2 – Wage and salary income 
  • 1099-NEC / 1099-MISC – Self-employment or contract income
  • 1099-K – Payment apps (Cash App, Zelle, PayPal, Venmo, Stripe, Square, etc.)
  • 1099-INT – Interest income
  • 1099-DIV – Dividends
  • 1099-R – Retirement distributions (pensions, IRA, 401(k))
  • SSA-1099 – Social Security benefits
  • 1099-G – Unemployment or state tax refunds
  • K-1 – Partnership, S-Corporation, or Trust income
  • Rental income records (income and expenses)

Self-Employment / Business Information (if applicable)

  • Income and expense summary (or bookkeeping records)
  • Mileage log (business use of vehicle)
  • Business expenses:
    • Supplies
    • Advertising
    • Office expenses
    • Vehicle expenses
    • Insurance
    • Utilities
  • Home office expenses (if applicable)
  • Asset purchases (equipment, vehicles, machinery)

Adjustments & Deductions

  • Student loan interest (Form 1098-E)
  • IRA contributions
  • Health Savings Account (HSA) contributions (Form 5498 / 1099-SA)
  • Alimony paid (for agreements before 2019)

Itemized Deduction Documents (if applicable)

  • Mortgage interest (Form 1098)
  • Property tax statements
  • State and local tax payments
  • Charitable donations (cash and non-cash)
  • Medical expenses (out-of-pocket)
  • Casualty or theft loss documentation

Credits

  • Childcare expenses (provider name, EIN/SSN, amount paid)
  • Education expenses:
  • Form 1098-T
  • Tuition receipts
  • Student loan statements
  • Adoption expenses (if applicable)

Other Important Documents

  • Prior year tax return (if new client)
  • IRS letters or notices (if any)
  • Estimated tax payments made
  • Foreign income or foreign bank account information (FBAR, if applicable)
  • Cryptocurrency transactions (buy/sell/trade records)


Required Documents for Returning Clients

For clients who have previously filed their income tax return with our firm

Personal Information

 

  • New or renewed government-issued photo ID
     
  • Changes to:
  •  Legal name
  • Marital status
  • Address
  • Phone number or email


  • Updated bank account and routing number

Dependents (Only If Changed)

 

  • New dependents (birth, adoption, or new qualifying dependent)
  • Social Security card or ITIN for new dependents
  • Changes in custody or living arrangements

Income Documents (Current Tax Year Only)

Provide all forms received, including:

  • W-2
  • 1099-NEC / 1099-MISC
  • 1099-K (payment apps: Cash App, Zelle, PayPal, Venmo, Square, Stripe)
  • 1099-INT / 1099-DIV
  • 1099-R (retirement income)
  • SSA-1099 (Social Security)
  • 1099-G (unemployment or state refunds)
  • K-1 (partnership, S-corp, trust)
  • Rental income and expense summary
  • Alimony received (pre-2019 agreements)

Self-Employment / Business (If Applicable)

  • Updated income and expense records
  • Mileage log (if claiming vehicle use)
  • New equipment or asset purchases
  • Home office changes (if applicable)

Adjustments, Deductions & Credits

  •  Student loan interest (Form 1098-E
  • Mortgage interest (Form 1098)
  • Property taxes paid
  • Charitable contributions
  • Childcare expenses (provider info required)
  • Education expenses (Form 1098-T)
  • IRA, HSA, or retirement contributions
  • Medical expenses (if itemizing)

Other Important Items

  • Cryptocurrency transactions (buy/sell/trade)
  • Foreign income or foreign bank accounts (if applicable

If nothing has changed from prior years, please still provide all income documents for the current tax year to ensure accurate filing.
